An application must be submitted for each person 18 and over. We recommend you fax or mail your application. Our fax number is 515-232-9721. If your application is received and all parties decide to move forward, we do charge $25 to run credit and background for each applicant. Payment can be made via check made out to “DPM”, or through our online payment option.
